Contact tracing form

To be filled in before each session

We are currently operating under a UQ Sport approved COVID-Safe plan. At this stage, only members will be allowed to attend sessions. All 2020 memberships are still valid. More details will be discussed at our AGM on Sunday 20 September 2020.

Please be aware of the session conditions below. Participation in our sessions means acceptance of these conditions. We ask for your cooperation and please follow our session managers' instructions at all times.

Session Conditions

Before you come to a session
Players should not attend any table tennis sessions if they:

  • have ay flu-like symptoms;

  • have been in direct contact with a known case of COVID-19 in the past 14 days;

  • have travelled internationally or interstate and have not yet quarantined for a full 14 days;

  • are at high risk from a health perspective, including the elderly and those with pre-existing medical health conditions.

Before you arrive at a session

  • Shower if necessary and get changed at home. UQ Sport change rooms are only for limited use.

  • Bring only what is necessary to play.

When arriving at a session

  • Use hand sanitiser provided on entry.

  • Register your attendance on our form for contact tracing purposes

  • The session manager will assign you a waiting area and chair. Keep all personal possessions there

During a session

  • Clean tables and any other equipment using disinfectant spray provided before and after play.

  • Use hand sanitiser before and after play.

  • Do not share personal equipment e.g. bats, towels, water bottles.

Before leaving a session

  • Clean your table and personal equipment using disinfectant spray provided.

After coming to a session
Players have the responsibility to report to the club in the following circumstances:

  • If you have any flu-like symptoms after attending a session and require testing; and

  • If you have tested positive for COVID-19; or

  • If you have been in contact with someone who has returned a positive test for COVID-19.
